Classroom Accommodation(s) Request

Requirements:

  1. Must have a permanent disability or temporary injury/condition.
  2. Currently enrolled at UCO.

Documents required to register:

  • Photo ID
  • Class schedule for the semester you are requesting accommodations.
  • Disability Documentation when new to DSS or been over two years since registered.

Please Note: Requests may not be processed until all necessary information is received. Once received, DSS will review the information and then contact you to schedule a time to visit with a DSS Coordinator. At this meeting, you and the Coordinator will engage in an interactive process to determine what, if any, classroom accommodations are needed. Please have your Student ID with you at time of your meeting. If you need accommodation(s) for this meeting, let our office know at the time of scheduling.  Once you apply, you will receive an email confirmation. Requests are processed in the order they are received - with priority given to current semester.

Housing Accommodation(s) Request

Requirements:

  2. Currently accepted to UCO.

Please Note: Requests may not be processed until all necessary information is received. Once received, DSS will review the information and then contact you to schedule a time to visit with a DSS Coordinator. At this meeting, you and the Coordinator will engage in an interactive process to determine what, if any, housing accommodations are needed. Please have your Student ID with you at time of your meeting. If you need accommodation(s) for this meeting, let our office know at the time of scheduling.  Once you apply, you will receive an email confirmation. Requests are processed in the order they are received - with priority given to current semester. Per University policy, requests for an emotional support animal should be submitted at least 60 days in advance of your move-in date.

Residual ACT/Placement Tests Accommodation Request Process:

If you need accommodations for the Residual ACT, Placement Tests, etc., please call (405) 974-2516 to set an appointment. You must bring documentation of your disability to the meeting. Documentation should meet our Documentation Guidelines or have your provider complete the online Disability Verification Form. If you need accommodation(s) for the appointment, let our office know at the time of scheduling.
