Important Update:

Consistent with revised CDC guidance, vaccinated individuals are recommended to wear a mask when in public indoor spaces where COVID-19 virus transmission is considered substantial or high. Oklahoma County is currently listed as an area of high transmission by the CDC. The CDC further recommends that unvaccinated individuals continue to wear a mask when around others indoors, regardless of their area’s rate of transmission.

Prospective UCO Student 

New /Current Students (Are a current student and now need accommodation)

Returning Students (Has registered with DSS within the past two years)

Temporary Injury/Condition accommodation request

 

Prospective UCO Student:

Potential UCO students and their families are welcome to schedule an appointment with a DS Provider to learn more about our services and the registration process. You can schedule an appointment by calling the office at 405-974-2516. Students needing any accommodation(s) for the appointment should let our office know at the time of scheduling.

Individuals needing accommodations for the Residual ACT, Placement Tests, etc., call (405) 974-2516 to set an appointment with a DSS provider.  Bring to your appointment: Disability Documentation.  You may submit medical documentation that meets our Documentation Guidelines or have your doctor complete the Disability Verification Form

New/Current Students  

1) Complete the Student Disability Registration Application (SDRA).  Documents required to upload while completing SDRA:


2)  Once the SDRA and documentation has been submitted, it will be reviewed by a DSS Provider to determine eligibility for accommodations/services.  Please note the review process may take up to 30 days.  Once eligibility is determined, you will be contacted by DSS to schedule a time to visit with a DSS Provider who will engage in an interactive process with you to determine accommodations. Please have your Student ID available when visiting DSS.

NOTE: Once you complete the SDRA, you will receive an email confirmation that your registration application has been submitted.

Returning Students 

Students who have been, registered with DSS with accommodations within the past two years, follow the below steps:

1) Complete the Returning Student Registration Application (RSRA). Documents required to upload while completing RSRA:

  •  Documentation: You may submit any updated medical documentation that meets our Documentation Guidelines or have your doctor complete the Disability Verification Form as needed. (optional)
  • Student schedule (if needing academic accommodations). Application has instructions on where in UConnect to get your schedule.

2)  Once the RSRA has been submitted, it will be reviewed by a DSS Provider to determine the continuation of accommodations/services.  Please note the review process may take up to 30 days during which time you will be contacted by DSS regarding your request for accommodations.  

NOTE: Once you complete the RSRA, you will receive an email confirmation that your registration application has been submitted.

Temporary Accommodations for Injury and/or Conditions

There are two steps to register with Disability Support Services.

1) Complete the Student Temporary Accommodation Application (STAA).   

Documents required to upload while completing STAA:

  • Documentation:  For guidance on documentation required, review our Documentation Guidelines.  You may submit medical documentation signed by your doctor on letterhead or have your doctor complete the online Temporary Injury and/or Condition Verification Form.
  • Student schedule (if needing academic accommodations). Application has instructions on where in UConnect to get your schedule.

2)  Once the STAA has been submitted, it will be reviewed by a DS Provider to determine eligibility for accommodations/services. Once eligibility is determined, you will be contacted by DSS.
