Multimedia Setup Request Form

Multimedia setup services are available to UCO departments at no charge with the following exceptions. Click here for more information.

Non-UCO events (this includes requests from UCO staff and departments for non-UCO events)
Any event, whether for UCO or not for UCO purposes, that takes place on the weekend.
Large events requiring setup in more than one room or location.

*Click here to confirm:		This agreement is made between the individual below (Borrower) and the Technology Resource Center (TRC) at the University of Central Oklahoma. TRC agrees to loan the following individual the equipment indicated above in this form. Borrower agrees to care for the equipment based on instructions from TRC Media Specialists and further agrees that he/she is personally responsible for any loss or damage to the aforementioned equipment.
