Multimedia Setup Request Form

Multimedia setup services are available to UCO departments at no charge with the following exceptions below:

  1. Non-UCO events (this includes requests from UCO staff and departments for non-UCO events)
  2. Any event, whether for UCO or not for UCO purposes, that takes place evening or weekend.
  3. Large events requiring setup in more than one room or location.

Please fill out the Multimedia Request Form to request equipment for your next meeting.

  Power Point Presentation (includes Laptop, Projector, Wireless Presenter)
  Web Conference (includes Laptop, Projector, Speakers, Audio Conferencing Phone if needed)
  Teleconference (includes Audio Conferencing phone)
  Display Video with Projector (includes Portable 60"x60" screen with Projector, PA system, VHS/DVD combo)
  Mobile Training Laptop Stations (consists of 15 Dell Laptops for use with UCO Training Services)
  This agreement is made between the individual below (Borrower) and the Technology Resource Center (TRC) at the University of Central Oklahoma. TRC agrees to loan the following individual the equipment indicated above in this form. Borrower agrees to care for the equipment based on instructions from TRC Media Specialists and further agrees that he/she is personally responsible for any loss or damage to the aforementioned equipment.
