Reporting Lost or Stolen Cards

The Central ID Visa Debit Card is issued by MidFirst Bank, member FDIC. Use of the card is subject to the terms of the agreement, as amended, under which it was issued. For issues, or to report a lost or stolen card, please contact MidFirst Bank, 24 hours a day, at 888-MIDFIRST (643-3477) and UCO, during normal business hours, at 405-974-2357.

This card is also governed by the regulations of the University of Central Oklahoma. It must be presented to secure specific services and privileges. It is nontransferable. A replacement fee will be charged for lost or mutilated cards.

The Bronze Card is governed by the regulations of the University of Central Oklahoma. It must be presented to secure specific services and privileges. It is nontransferable. This card remains the property of the University of Central Oklahoma and must be surrendered to an authorized university official upon request.

Stolen Bronze or Central Cards will be replaced without charge if the student can provide a copy of a police incident report.
